The following worksheet provides the basic information required to provide an evaluation sample. Please contact our sales department at (909) 305-2363 if you require assistance completing the worksheet.

Our policy regarding samples is we will provide the first sample of a standard catalog single valve at no charge. Additional samples, manifolds or custom products will be quoted based on the individual application.

Alcon will pay ground shipping costs for the sample. Special shipping requests or air shipments will be at the customers expense. All shipping costs are to be paid by customers located outside the USA..
